In Short

• The Clinical Consultant is responsible for Clinical Oversight of ECM Lead Care Managers.
• Communicates directly with ECM Lead Care Managers and occasionally with patients.
• Provides guidance during emergency situations encountered by ECM Lead Care Managers.
• Collaborates with the client’s broader care team.
• Participates in developing patient-centered care plans.
• Provides health and preventive care education.
• Engages vulnerable populations as part of a multidisciplinary outreach team.
• Helps to expand programming that addresses social determinants of health.
• Works with hospitals to coordinate admission/discharge plans.
• Performs timely medication reconciliation following transitions in care.

Requirements

• LCSW licensed in California.
• Two to three years of care management experience.
• Strong organizational skills.
• Ability to work independently.
• Proficient with Microsoft and Google applications.
• Strong communication skills.
• Strong motivational skills including conflict/dispute resolution techniques.
• Must have access to reliable internet connection.
• Willing to work Monday-Friday 8:30am-5:00pm Pacific Time.
• Proven results-driven business leader.
