This role involves supporting cancer patients through clinical assessments, care coordination, and education, aiming to improve their healthcare journey.

In Short

  • Conduct clinical assessments and monitor health changes.
  • Coordinate care transitions for oncology patients.
  • Educate members and caregivers about diagnoses and treatments.
  • Provide telephonic assessments and monitor member progress.
  • Build relationships with members, payers, and providers.
  • Assist with care management following admissions.
  • Perform virtual home safety evaluations.
  • Available for urgent clinical escalations.
  • Work within a member-first approach.
  • Utilize technology for effective communication and documentation.

Requirements

  • Bachelor of Science Degree in Nursing (BSN).
  • Unrestricted Registered Nurse (RN) license.
  • At least 5 years of nursing experience.
  • 3 years of oncology nursing or case management experience.
  • Certified Case Manager (CCM) certification.
  • Strong organizational skills.
  • Effective listening and communication skills.
  • Comfort with ambiguity in fast-paced environments.
  • Desire to learn new technologies.
  • Ability to prioritize member needs.
