Overview

The Social Care Navigator role at Tangelo focuses on supporting members in New York, particularly those facing social, behavioral, or environmental challenges impacting their health. You will utilize Motivational Interviewing techniques to assist members in setting goals and navigating community resources.

In Short

• Support members facing health-related challenges.
• Use Motivational Interviewing to build trust.

Requirements

• 2+ years in Care Navigation, Social Work, or related role.
• Fluency in Spanish required.
• Skilled in Motivational Interviewing.
• Familiar with Medicaid, SNAP, and public housing.
• Experience with low-income individuals or chronic conditions.
• Comfortable using digital tools for organization.
