In Short

• Use motivational interviewing to build trust and empower members.

Requirements

• 2+ years of experience in Care Navigation or related roles.
• Fluency in Spanish required.
• Skilled in Motivational Interviewing.
• Familiar with Medicaid, SNAP, and public housing.
• Experience with low-income individuals or chronic conditions.
• Comfortable working remotely with digital tools.
• Occasional travel for in-person training may be required.
