Overview

The Social Care Navigator role at Tangelo focuses on supporting members in New York by helping them navigate social, behavioral, or environmental challenges impacting their health and well-being.

In Short

• Use motivational interviewing techniques to empower members.
• Conduct outreach via phone, text, or video to understand member needs.
• Identify and refer members to local resources and services.
• Collaborate with internal teams for continuity of care.
• Document member interactions and care plans accurately.
• Follow up with members to check on progress and troubleshoot barriers.
• Provide trauma-informed and culturally responsive care.
• Contract role requiring 35 hours per week.

Requirements

• 2+ years experience in Care Navigation, Social Work, or related roles.
• Fluency in Mandarin or Cantonese is required.
• Skilled in Motivational Interviewing or similar approaches.
• Familiar with systems like Medicaid and SNAP.
• Experience with low-income individuals or chronic conditions.
• Comfortable working remotely and using digital tools.
• Occasional travel may be required for in-person training.
