Overview

The Social Care Navigator will support members in New York, particularly those facing social, behavioral, or environmental challenges affecting their health and well-being, using Motivational Interviewing techniques.

• Experience with low-income individuals or chronic conditions.
• Comfortable with remote work and digital tools.
