Dementia Care Nurse Practitioner - Remote

Overview

As a Dementia Care Nurse Practitioner at Synapticure, you will provide comprehensive care for patients with dementia and other neurodegenerative diseases through telehealth services, collaborating with a team to enhance patient and caregiver experiences.

In Short

• Diagnose, treat, and manage dementia and acute episodic illnesses.
• Conduct telehealth visits with patients and caregivers.
• Facilitate annual visits to monitor dementia care plans.
• Provide education and management skills to caregivers.
• Order and interpret lab and diagnostic testing.
• Collaborate with neurologists and interdisciplinary teams.
• Maintain documentation in the electronic medical record (EMR).
• Support strategic initiatives related to dementia care.
• Ensure compliance with state and federal regulations.
• Assist in developing training materials for diverse audiences.

Requirements

• Masters of Science in Nursing or equivalent.
• Current Nurse Practitioner certification.
• Active, unrestricted APRN licensure.
• 2 years of APRN experience in relevant fields.
• Knowledge of geriatrics and neurology.
• Excellent communication and organizational skills.
• Experience with EMR and communication platforms.
• Ability to work in a fast-paced, team-based environment.
• Experience in assessing psychosocial needs of patients.
• Bilingual in Spanish is preferred.
