Overview

This job provides primary or acute healthcare services for certain acute or chronic health conditions in specific populations in accordance with applicable scope and standards of practice and with the policies, values, and mission of the organization.

In Short

• Provides appropriate patient care including recording medical history and physical assessment.
• Develops therapeutic care plans and orders diagnostic tests.
• Communicates and coordinates patient care with family and interdisciplinary teams.
• Prepares written prescription orders and administers medications.
• Educates patients and families on health and preventive medicine.
• Triages patient telephone calls and provides continuity of care.
• Participates in quality improvement initiatives.
• Acts as a clinical expert and resource.
• May perform daily rounding of patients.
• Ensures compliance with regulatory agencies.

Requirements

• Minimum MSN from an accredited nursing program.
• National Board Certification for Nurse Practitioners.
• Practicing-state licensure as a Nurse Practitioner.
• Prescriptive Authority within 60 days of hire.
• Drug Enforcement Agency Licensure within 60 days of hire.
• CPR certification from the American Heart Association.
• Act 34, 33, and 73 Clearances.
• Preferred: Post Master’s program or DNP.
• Minimum 3 years in family medicine as CRNP.
• EPIC experience preferred.
