Patient Navigator - Remote

Overview

The Patient Navigator is responsible for supporting Strive’s care team to keep patients engaged with our complete care program specifically designed for people with kidney conditions. While most of your patient interactions will come by phone, there will be opportunities to meet patients face to face at their office visits or in their home. Success is measured by scheduling patients for program appointments which will occur through building strong relationships, being an educational resource, and helping navigate questions.

• Proactively outreach to both current and prospective patients via phone.
• Build trusting, long-term relationships with patients.
• Meet or exceed daily outreach expectations.
• Present yourself as an educational and empathetic resource.
• Visit with patients in-person, when possible.
• Maintain up-to-date client information in CRM tools.
• Meet with partner nephrologists and practice staff.
• Face-to-face patient outreach at various locations.
• Maintain long-term relationships with patients.
• Travel required to key markets within your territory.

Requirements

• High School Diploma.
• 5+ years combined education or experience in a professional setting.
• Efficient and reliable transportation with an active driver’s license.
• Provides in-person patient care which may include physical activity.
• Internet Connectivity - Min Speeds: 3.8Mbps/3.0Mbps (up/down).
• Business travel required up to 80%, some overnights.
