Patient Navigator RN - Remote

Overview

The Patient Navigator RN will assist patients in understanding their diagnosis, coordinate treatment plans, and communicate with healthcare providers, while working primarily remotely with some in-office meetings.

• Work Monday to Friday, day shift.
• Assist in assessment, planning, implementation, and evaluation of nursing care.
• Guide patients through their healthcare experience.
• Ensure all treatment options are explored from diagnosis to care completion.
• Maintain updated medical records and communicate with healthcare teams.
• Collaborate with surgical scheduling and prepare patients for procedures.
• Attend educational workshops to maintain professional knowledge.
• Comply with HIPAA regulations for patient confidentiality.

Requirements

• Graduate of an accredited nursing program required; BSN preferred.
• 2 years of previous clinical experience.
• RN License and certification as required by state.
• BLS certification required.
• Excellent verbal and written communication skills.
• Strong organizational skills and attention to detail.
• Ability to build relationships with patients.
• Knowledge of healthcare systems operations.
• Skill in using computer programs including Microsoft Office.
