RN Nurse Navigator - Remote

Overview

The RN Nurse Navigator for AOC’s Health Home Services team will be directly supporting AOC’s Star and Star Kids clients, responsible for organizing, coordinating, and providing care coordination and care management services.

• Support AOC’s Star and Star Kids clients.
• Organize and coordinate care management services.

• Maintain comprehensive clinical records.

• Research and recommend resources for clients.

Requirements

• Registered Nurse license required.
• Minimum of 2 years Case Management experience.
• Minimum of 2 years clinical experience in pediatric care.
• Knowledge of case management and resource management.
• Excellent verbal and written communication skills.
• Ability to monitor and assess patient progress.
• Ability to work collaboratively with medical staff.
• Ability to maintain confidentiality of client information.
• Ability to adapt to changing environments.
• Physical ability to participate in training and meetings.
