Nurse Practitioner (Hybrid Remote)

Overview

The Nurse Practitioner (NP) at Strive Health will provide advanced nursing care for patients with chronic kidney disease and end-stage kidney disease, working within an interdisciplinary team to improve patient outcomes.

In Short

• Provide in-person and virtual patient care.
• Collaborate with a team of healthcare professionals.

Requirements

• Master’s degree in Nursing or related field.
• 3+ years of experience as a Nurse Practitioner.
• Current state licensure and national board certification.
• Autonomous Practice certification preferred.
• BLS or CPR Certification required.
• Efficient transportation for patient visits.
• Internet connectivity for telehealth services.
• Basic computer skills and EHR experience preferred.
• Demonstrated proficiency in clinical assessment and patient management.
• Excellent communication and teamwork skills.
