• Communicate with veterans to gather necessary information prior to service

• Collect and upload medical records to clinical systems
• Confirm appointment logistics for veterans
• Monitor CRM/EHR task queues for timely resolution
• Serve as the primary contact for veterans navigating the intake-to-service process
• Contribute to streamlined care coordination workflows
• Support initiatives to expand services for veterans and their families

Requirements

• 2+ years of experience in healthcare coordination or similar clinical operations
• Strong understanding of medical documentation and patient communication
• High attention to detail and ability to manage multiple cases
• Experience with CRM, EHR, or care management platforms
