Care Support Partner - Remote

Overview

The Care Support Partner will serve as a primary relationship holder within the Care Delivery Service Model, providing personalized guidance to clients, families, and caregivers throughout their journey.

• Serve as the primary relationship holder for clients, families, and caregivers.
• Conduct thorough intake assessments to identify needs and goals.
• Develop individualized service plans leveraging community resources.
• Coordinate service delivery and manage data in the client portal.
• Use motivational interviewing and trauma-informed care to build trust.
• Conduct follow-ups to ensure continuity of care.
• Participate in team meetings and case reviews.
• Communicate cross-functionally to ensure seamless service delivery.

Requirements

• Bachelor’s degree in Social Work, Human Services, or related field.
• 3-5 years of direct service experience in a person-centric environment.
• Experience in remote care coordination and case management.
• Commitment to serving diverse populations with empathy.
