In Short

  • Engage with cancer patients via phone, text, video, and email.
  • Assist members in connecting with healthcare providers and resources.
  • Schedule health and wellness visits for members.
  • Conduct survey assessments to evaluate member needs.

  • Complete virtual onboarding and training within three weeks.
  • Build trusting relationships with members through effective communication.
  • Identify and prioritize member needs for assistance.
  • Follow Care Team policies and ensure quality standards are met.
