The Community Care Partner engages members in their communities and offers navigation support through their cancer journey, working both in-person and virtually.

In Short

• Engage members in person and virtually.

Requirements

• 2+ years in a patient-facing role.
• Experience in care coordination or community health.
• Community Health Worker Certification preferred.
• Strong communication and interpersonal skills.
• Ability to build rapport quickly.
• Organized with client material and appointment tracking.
• Flexible and adaptable to changing needs.
• Comfort with technology and remote work.
• Passionate about assisting clients.
• Ability to identify and prioritize needs.
