Overview

The Care Coach for Chronic Care Management (CCM) is responsible for supporting patients remotely, developing personalized care plans, and engaging with patients to improve healthcare delivery.

In Short

• Develop personalized CCM care plans for patients.
• Serve as the point of contact for patients enrolled in the RPM365 platform.
• Engage regularly with patients to encourage treatment consistency.
• Schedule monthly check-ins with patients.
• Educate patients through shared information.
• Assess and provide resources for Social Determinants of Health.
• Attend virtual meetings for updates.

Requirements

• Possess a Nursing License in NJ and/or NY.
• Knowledgeable about chronic health conditions.
• Proficient with technology.
• Comfortable navigating computer platforms and smartphones.
• Excellent communicator (written and verbal).
• Highly organized and efficient.
• Detail-oriented and good at multi-tasking.
• Team player with self-motivation.
