The Community Health Care Team Manager is responsible for leading a team that provides support to individuals with serious mental illness and substance use disorders, ensuring effective communication and collaboration with clinical staff.

  • Establish communication structures and accountability processes.
  • Review performance and provide feedback to Community Guides.
  • Collaborate with clinicians for high-quality service delivery.
  • Facilitate team meetings and check-ins.
  • Support and supervise Community Guides.
  • Participate in hiring and onboarding processes.
  • Ensure compliance with HIPAA and confidentiality policies.
  • Build relationships with local community partners.

Requirements

  • Bachelor’s degree in Social Work, Human Services, or related field.
  • Three years of management experience in healthcare or mental health.
  • Five years of direct experience with the local community.
  • Proficiencies in critical thinking and time management.
  • Knowledge of crisis planning and behavioral health.
  • Experience with diverse populations and recovery principles.
  • Ability to navigate ambiguity and problem-solve.
  • Strong relationship-building skills.
  • Experience with Microsoft Office and video conferencing tools.
