In Short

• Join our cardiac recovery program as a Cardiac Rehab Intake Specialist.
• Conduct initial assessments for patients enrolling in the virtual program.
• Educate patients about program benefits and risk factor modification.
• Support patients through electronic messaging and video sessions.
• Complete individualized treatment plans for patients.
• Motivate and encourage patients during their initial visits.
• Work collaboratively with exercise physiologists and other teams.
• Adapt in-clinic practices to a remote care delivery model.
• Utilize evidence-based practices for chronic condition management.
• Foster a supportive and inclusive culture within the team.

Requirements

• Minimum of three years of experience in health management.
• Degree in a health-related field.
• Experience in counseling or education of disease management.
• Passion for helping people change their lives.
• Detail-oriented and team-first mindset.
• Fluent in English; bilingual in Spanish is a plus.
• Some experience in a clinical setting.
• Knowledge of cardiac rehabilitation.
• Experience with elderly patient populations.
• Bonus points for experience in smoking cessation and diabetes management.
