Systems Navigator - Remote

Overview

We are seeking a Systems Navigator to join our Behavioral Health Home team! This program utilizes a multi-disciplinary team to deliver person-centered services designed to support a person in coordinating care and services while reaching their health and wellness goals.

In Short

• Full-Time position, Monday-Friday, 8am-4:30pm
• Hybrid work model with remote opportunities
• Act as the central point of contact for clients
• Coordinate care between behavioral and physical health specialists
• Educate clients on available resources
• Maintain accurate case notes and document client activities
• Assess client needs for individual development plans
• Collaborate with a multi-disciplinary mental health team
• Promote recovery, stability, and self-sufficiency
• Hiring bonus of $1,000 for external candidates

Requirements

• Minimum age of 21 years
• Ability to pass all background checks
• Excellent driving record and reliable car
• Master’s or Bachelor’s degree in behavioral science or related field
• Experience in service delivery to individuals with mental illness
• Fluency in a non-English language is a plus
