Overview

Pair Team is seeking a highly motivated full-time Behavioral Health Care Manager to provide mental health resources and interventions for individuals with Serious Mental Illness and Substance Use Disorders, primarily working remotely with occasional community visits.

In Short

  • Work with individuals struggling with severe mental illness or substance use disorders.
  • Provide evidence-based short-term interventions and behavioral strategies.
  • Assist individuals in linking to ongoing behavioral health treatment.
  • Educate on behavioral health conditions to improve health literacy.
  • Collaborate with a multidisciplinary team to enhance patient care.
  • Track and manage caseloads using electronic medical records.
  • Conduct crisis intervention techniques as needed.
  • Engage in community-based activities 1-2 times a month.
  • Maintain a HIPAA compliant and internet-connected workspace.
  • Support ECM team’s caseload and address barriers to care.

Requirements

  • LCSW, LMFT, or LPCC license issued by the state of California.
  • Bilingual English/Spanish.
  • Strong understanding of cultural fluency.
  • Experience working with individuals with complex needs.
  • Technical skills with CRM databases and video conferencing.
  • Located in or near San Diego, California.
