Behavioral Health Care Manager - Remote

Overview

The Behavioral Health Care Manager (BHCM) is a core member of Cerula Care’s collaborative care team, focusing on providing integrated behavioral health services to individuals living with cancer.

In Short

• Engage with members through telehealth.
• Coordinate care between external specialists and the internal care team.
• Conduct clinical member biopsychosocial intakes and assessments.
• Lead interdisciplinary team meetings.
• Document member progress in a HIPAA-compliant system.
• Identify patients needing more intensive mental health care.
• Facilitate referrals for clinically indicated services.
• Work primarily remote with some in-person requirements.
• Part-time, hourly contractor position.
• Start date on 2/1/25.

Requirements

• Bachelor’s Degree in nursing, social work, or related field.
• Experience in clinical care management, preferably with cancer patients.
• Certification as a Case Manager or similar is preferred.
• Strong organizational skills and adaptability.
• Passion for helping members with cancer.
