Overview

The Behavioral Health Care Manager (BHCM) is a core member of Cerula Care’s collaborative care team, focusing on providing integrated behavioral health services to individuals living with cancer.

In Short

  • Engage with members through telehealth.
  • Coordinate care between external specialists and the internal care team.
  • Conduct clinical member biopsychosocial intakes and assessments.
  • Lead interdisciplinary team meetings.
  • Document member progress in a HIPAA-compliant system.
  • Facilitate referrals for additional services.
  • Provide brief interventions and follow-up visits.
  • Identify patients needing more intensive care.
  • Maintain organization of care coordination tasks.
  • Adapt to a start-up environment.

Requirements

  • Bachelor’s Degree in nursing, social work, or related field.
  • Experience in clinical care management, preferably with cancer patients.
  • Certification as a Case Manager or similar is preferred.
  • Strong organizational skills.
  • Ability to engage members effectively.
  • Experience in telehealth is a plus.
  • Interest in health coaching and wellness.
  • Ability to work collaboratively in a team.
  • Adaptability to a dynamic work environment.
  • At least one year of relevant experience.
