Bilingual Case Manager (Remote Flexible, Spanish Speaking)

In Short

• Full-time Lead Care Manager position.
• Remote work with occasional on-site visits in the community.
• Engage with individuals living with Serious Mental Illness/Substance Use Disorder.
• Utilize evidence-based approaches to promote health goals.
• Support members with social support navigation.
• Conduct telephonic and SMS outreach for follow-up.
• Collaborate with a multidisciplinary team.
• Meet monthly encounter metrics for compliance.
• Identify barriers to program continuation.
• Assist individuals in securing community support connections.

Requirements

• 1+ years of general work experience (Case Management preferred).
• Bilingual in English and Spanish.
• Strong understanding of cultural fluency.
• Empathy and collaborative skills.
• Organizational skills and ability to multitask.
• Strong technical skills with CRM databases.
• Valid driver’s license and auto liability insurance.
