Bilingual Spanish Care Manager - Remote

Overview

The Care Manager plans and manages behavioral and/or physical care with members, working collaboratively to ensure an excellent member experience and improve health outcomes.

In Short

• Advocates and educates beneficiaries on health services.
• Conducts assessments to identify barriers to care.
• Develops care plans aligning with physician treatment plans.
• Generates referrals to providers and community resources.
• Liaises between service providers to ensure coordinated care.
• Documents encounters and updates care plans as needed.
• Evaluates service authorizations for alignment with care plans.
• Assists in identifying alternative care options.
• Contributes to corporate goals through execution of care plans.
• May require occasional overtime.

Requirements

• For Medical Care Management: NYS RN or LCSW or LMSW.
• 3 years of experience in a mental/behavioral health setting.
• 1 year of pediatric clinical field experience for PEDS positions.
• Fluency in Spanish is preferred.
• Strong interpersonal and assessment skills.
• Experience managing large caseloads.
• Proficient in using electronic documentation systems.
• Knowledge of community health practices for seniors.
• Relevant work experience as a Care Manager is a plus.
