In Short

  • Remote work with 1-2 on-site visits per month.
  • Engage with individuals living with Serious Mental Illness/Substance Use Disorder.
  • Utilize evidence-based approaches for health goal achievement.
  • Support members with social support navigation.
  • Conduct telephonic and SMS outreach for timely follow-ups.
  • Collaborate with multidisciplinary teams.
  • Maintain compliance with health plan regulations.
  • Identify barriers for program continuation.
  • Assist individuals in securing community support connections.
  • Flexible vacation policy and comprehensive health benefits.

Requirements

  • 1+ years of general work experience (Case Management preferred).
  • Bilingual in English and Spanish.
  • Strong understanding of cultural fluency.
  • High degree of empathy and organizational skills.
  • Technical skills with CRM databases and Microsoft Office.
  • Valid driver’s license and auto liability insurance.
