• Full-time Lead Care Manager position focused on high-need patients.

• Conduct telephonic and SMS outreach for follow-ups.

• Assist individuals in connecting to community supports.
• Utilize online platforms for collaboration and task execution.

Requirements

• 1+ years of general work experience, preferably in Case Management.
• Bilingual in English and Spanish.
• Strong understanding of cultural fluency and empathy.
• Ability to work collaboratively in a team.
• Strong technical skills with CRM databases and MS Office.
• Organizational skills and patience in adversity.
• Valid driver’s license and auto liability insurance.
