Remote Admissions Coordinator – Long-Term Care & Rehabilitation (RN or LVN/LPN)

Overview

An experienced and compassionate nurse is needed to manage admissions into long-term care and rehabilitation programs in a fully remote role for a managed care organization serving California Medi-Cal members.

• Coordinate admissions and transitions of care for long-term care and rehabilitation.

Requirements

• Active California RN or LVN/LPN license.

• Strong communication and problem-solving skills.
• Proficiency with EMRs and care coordination platforms.
• Ability to work independently in a remote environment.
