Overview

We are hiring a Part Time LPN/LVN Care Coordinator to work within our contact center, managing day-to-day functions and ensuring patients receive the care they need.

In Short

  • Part Time LPN/LVN Care Coordinator role.
  • Fully remote position (15-20 hours a week).
  • Interface directly with patients, providers, and families.
  • Educate patients by phone and schedule palliative appointments.
  • Work with clinical files and document palliative visits.
  • Strong multitasking skills required.
  • 3+ years as an LPN and customer service experience needed.

  • Regular communication via Zoom and Slack.
  • Spanish-speaking is a plus.

Requirements

  • Minimum 3+ years as an LPN.
  • Experience in reading and updating clinical notes.
  • 1+ years in a customer service call center.
  • Strong communication and written skills.
  • Detail-oriented with strategic thinking.
  • Comfortable working with technology.
  • Secure Wi-Fi connectivity.
  • Passionate about improving healthcare experiences.
  • Enthusiastic about serving patients and caregivers.
  • Ability to balance clinical triage and patient calls.
