Empassion is focused on improving the experience and lowering costs for seniors at end of life by providing palliative and hospice care. We are hiring a Part Time LPN/LVN Care Coordinator to work within our contact center, managing day-to-day functions and ensuring patients receive the care they need.

Requirements

• Minimum 3+ years as an LPN.
• Experience in reading and updating clinical notes.
• 1+ years in a customer service call center.
• Strong communication and written skills.
• Detail-oriented with strategic thinking.
• Comfortable working with technology.
• Secure Wi-Fi connectivity.

• Ability to balance clinical triage and patient calls.
