Care Manager RN with Field Travel - Remote

Overview

The Care Manager RN will support members by coordinating integrated care across various health services, ensuring quality and cost-effective care outcomes.

In Short

• Remote position with 30% field travel required.
• Supports Ohio state plan and serves members in the Cincinnati/Dayton area.
• Conducts comprehensive assessments and develops case management plans.
• Maintains ongoing member case load for outreach and management.
• Facilitates interdisciplinary care team meetings.
• Utilizes motivational interviewing techniques with members.
• Requires active RN license and previous care management experience.
• Promotes integration of services for members.
• Conducts medication reconciliation as needed.
• Work hours are Monday – Friday, 8:00 am – 5:00 pm EST.

Requirements

• Graduate from an Accredited School of Nursing.
• Bachelor's Degree in Nursing preferred.
• 1-3 years in case management or related fields.
• Active, unrestricted State RN license in good standing.
• Valid driver's license and reliable transportation.
• Preferred: 3-5 years in case management or related fields.
• Preferred: Active, unrestricted Certified Case Manager (CCM).
• Organizational and time management skills.
• Technologically proficient.
• Bilingual candidates encouraged to apply.
