Care Manager RN with Field Travel - Remote

Overview

The Care Manager RN position involves assessing, facilitating, planning, and coordinating integrated care for members with high needs, ensuring quality and cost-effective care.

In Short

• Support Ohio state plan members with care management.
• Requires RN licensure and previous care management experience.
• 30% field travel required.
• Work hours: Monday – Friday 8:00am – 5:00pm EST.
• Conduct comprehensive assessments and develop care plans.
• Facilitate interdisciplinary care team meetings.
• Promote integration of services for members.
• Use motivational interviewing techniques.
• Maintain ongoing member case load.
• Drive within applicable state/locality with reliable transportation.

Requirements

• Graduate from an Accredited School of Nursing.
• 1-3 years in case management or related fields.
• Active, unrestricted State RN license.
• Valid driver's license with a good driving record.
• Bachelor's Degree in Nursing preferred.
• 3-5 years in case management preferred.
• Active, unrestricted Certified Case Manager (CCM) preferred.
