Overview

As a Case Manager RN-Field at CVS Health, you will play a vital role in transforming healthcare for dual eligible populations, providing compassionate care and support while working from home with some travel within designated counties.

In Short

  • Work from home with 25-50% travel in Wayne and Macomb Counties.
  • Standard business hours: 8:00 AM - 5:00 PM.
  • Assess and coordinate care for members with complex health needs.
  • Utilize clinical tools to evaluate member needs.
  • Conduct holistic assessments and develop care plans.
  • Collaborate with interdisciplinary teams and community organizations.
  • Employ motivational interviewing techniques for member engagement.
  • Maintain compliance with regulatory and company policies.
  • Requires active MI RN license and relevant experience in care management.
  • Excellent communication and organizational skills are essential.

Requirements

  • Registered Nurse with active MI state license in good standing.
  • 3-5 years of clinical practical experience.
  • 2-3 years of experience in Care Management or related fields.
  • Behavioral Health experience preferred.
  • Ability to work independently and effectively from home.
  • Strong analytical and problem-solving skills.
  • Proficiency in standard corporate software applications.
  • Effective communication and interpersonal skills.
  • Ability to travel as needed for in-person activities.
  • Certified Case Manager is preferred.
