Case Manager - Remote

Overview

Our team is seeking a Case Manager to work with us in Florida, focusing on delivering our Population Health Program and supporting members in achieving better health outcomes.

In Short

• Conduct meaningful outreach and drive member enrollment.
• Create Service Plans based on member needs and diagnoses.
• Build measurable goals to manage health challenges.
• Coordinate community resources to support member goals.
• Act as an advocate between care teams and members.
• Schedule appointments and follow-up services.
• Report on hospital diversions and productivity.
• Maintain required records and reports.

Requirements

• Bachelor’s Degree in nursing, social work, or related field.
• Experience in case management or health services preferred.
• Certification as a Case Manager or Community Health Worker preferred.
• Knowledge of local resources and social issues required.
