Case Manager - Remote

Overview

The Case Manager will work closely with Mindoula’s members in the community to understand their health conditions and help them achieve better health outcomes.

In Short

• Conduct outreach and drive member enrollment.
• Create Service Plans based on member needs.
• Build measurable goals to reduce hospital admissions.
• Coordinate community resources for members.
• Act as an advocate between the care team and member.
• Schedule appointments and follow-up services.
• Report on hospital diversions and productivity.
• Maintain required records and reports.

Requirements

• Bachelor’s Degree in nursing, social work, or related field.
• Experience in case management preferred.
• Certification as Case Manager or Community Health Worker preferred.
• Knowledge of local resources and social determinants of health required.
