Community Supports Specialist - Remote

In Short

• Support Lakeland Care's Self-Directed Support Service model.
• Collaborate with Interdisciplinary Team (IDT) staff and service providers.
• Negotiate rates for service provider contracts.
• Monitor provider contract adherence and quality.
• Provide consultative expertise to support organizational goals.
• Drive innovative strategies to enhance community supports utilization.
• Evaluate initiatives using data for continuous improvement.
• Establish positive working relationships with stakeholders.
• Support care planning and community resources.
• Attend professional meetings and collaborate with team members.

Requirements

• Bachelor’s degree in a human service field or equivalent experience.
• Minimum of two years of experience in the Family Care program preferred.
• Strong mediation and negotiation skills.
• Knowledge of disabilities associated with LCI target groups.
• Creative problem-solving and critical thinking skills.
• Excellent organization and interpersonal skills.
• Proficiency in Microsoft Office (Word, Excel, PowerPoint, Outlook).
• Strong written and oral communication skills.
• Current driver’s license and proof of insurance required.
