Overview

The Field Service Coordinator assesses and evaluates members' needs and requirements to achieve and maintain optimal wellness by guiding members and families toward appropriate resources for care and wellbeing.

In Short

  • Conduct assessments to evaluate member needs and develop care plans.
  • Connect members with long-term services and supports.
  • Support members in navigating health and social services.
  • Collaborate with care teams to ensure cohesive service delivery.
  • Maintain Medicaid eligibility for members.
  • Utilize electronic applications for documentation and communication.
  • Meet members in-person and via telephone to assess needs.
  • Participate in Humana's TB screening program.
  • Travel occasionally for training or meetings.
  • Work full-time, 40 hours per week.

Requirements

  • Bachelor's degree in health and human services field.
  • 2+ years of related experience.
  • Knowledge of Microsoft Office Word and Excel.
  • Exceptional verbal and written communication skills.
  • Ability to use various electronic information applications.
  • Passion for improving consumer experiences.
  • Valid state driver's license and vehicle insurance required.
  • Preferred experience with Medicare & Medicaid recipients.
  • Experience with health promotion and wellness coaching.
  • Knowledge of community health and social service agencies.
