Case Manager (Community Resource Coordinator) - Remote

Overview

Humana is seeking a Case Manager, aka: Community Resource Coordinator (CRC), to join the Inclusa team to serve members in the Wisconsin Family Care (FC) program. In this role, you will be responsible for partnering with your fellow colleagues to provide top quality care to our members.

In Short

• Provide comprehensive social service care management for frail elders and adults with disabilities.
• Partner with Health and Wellness Coordinators to support member care plans.
• Conduct face-to-face assessments and coordinate care plans.
• Educate members on prevention/wellness and mitigate risks.
• Document activities and complete necessary paperwork.
• Travel required within Wisconsin, up to 40% of the time.
• Work from home with specific geography requirements.
• Participate in the ongoing review of member care plans.
• Engage in community integration and support non-health related issues.
• Must have a valid state driver's license and personal vehicle insurance.

Requirements

• Four-year bachelor’s degree in Human Services or related field.
• One year of experience with Family Care target populations.
• Preferred: Certification in social work.
• Strong documentation and communication skills.
• Ability to travel within designated areas.
• Experience in care management or social services.
• Knowledge of community resources and support systems.
• Commitment to providing quality care and support.
• Ability to work independently and as part of a team.
• Must meet internet connection requirements for remote work.
