Overview

The RN Nurse Navigator serves as a critical point of contact for patients and healthcare providers, coordinating care and ensuring quality services for high-risk patients within the Allegheny Health Network.

In Short

• Facilitates patient flow and coordinates appointments.
• Works with multidisciplinary teams to ensure comprehensive patient care.

• Minimum of 1-3 years nursing experience required.

Requirements

• Registered Nurse license required.
• 1-3 years nursing experience in specialty.
• CPR certification from the American Heart Association.
• Background clearance certificates as required.
• Bachelor's Degree in Nursing preferred.
• Valid PA driver's license may be required.
• Clearance under Older Adults Protective Services Act may be needed.
