Overview

The Patient Transition Coordinator facilitates and monitors the referral workflow from acute care to home, working closely with the Transitional Care Coordinator to meet patients' post-acute care needs.

In Short

  • Coordinate patient referrals from hospitals to home care.
  • Work with clinical teams to secure necessary documentation.
  • Handle equipment delivery workflows.
  • Engage with patients to obtain choices for post-acute care.
  • Build relationships with healthcare partners.
  • Respond to consumer inquiries regarding services.
  • Participate in process improvement efforts.
  • Complete mandatory on-site training in Pittsburgh.
  • Maintain accurate and timely documentation.
  • Perform miscellaneous duties as assigned.

Requirements

  • High school diploma or GED required.
  • Clinical or medical background preferred.
  • 1 year experience with Electronic Medical Records (EMR).
  • Strong data entry skills.
  • 1-3 years’ experience in the medical profession preferred.
  • Excellent interpersonal skills.
  • Project coordination experience.
  • Ability to work well with internal and external stakeholders.
  • Flexibility in work schedule.
  • Customer service experience preferred.
