Overview

The Health Outcomes Pharmacist is responsible for driving value creation and cost control in the Organization's value-based reimbursement programs. This role involves engaging with primary care providers and facilities to enhance clinical care models and improve performance metrics.

• Serve as a Clinical Pharmacy Subject Matter Expert (SME).
• Support primary care providers (PCPs) in value-based reimbursement programs.
• Analyze performance metrics for quality and cost.

Requirements

• Bachelor's Degree in Pharmacy required.
• 5 - 7 years of experience in Pharmacy.
• 3 - 5 years in a clinical practice.
• Preferred Master's Degree in Health Administration or Business.
• Experience in Medicare, Managed Care, and Quality Improvement is preferred.
• Licenses such as Board Certified Pharmacotherapy Specialist are preferred.
• Strong analytical skills required.
• Ability to deliver presentations effectively.
• Experience in healthcare management and care planning.
• Travel required 50% - 75% of the time.
