Care Manager - Bilingual Spanish/English - Remote

Overview

The Care Manager plans and manages behavioral and/or physical care with members and works collaboratively with them, their supports, providers, and health care team members to improve health outcomes.

In Short

• Responsible for applying care management principles.
• Conduct assessments to identify barriers and opportunities.
• Develop care plans aligning with physician’s treatment plans.
• Generate referrals to providers and community resources.
• Coordinate with multi-disciplinary health care teams.
• Document encounters with providers and members.
• Must be fluent in Spanish.
• Minimum qualifications include relevant licenses and experience.
• Experience managing large caseloads.
• Proficient in Microsoft Excel and electronic documentation systems.

Requirements

• NYS RN or LCSW or LMSW (any state).
• 3 years of work experience in a mental/behavioral health setting.
• 1 year pediatric clinical experience for PEDS positions.
• Strong interpersonal and assessment skills.
• Experience with community health practices.
• Ability to handle rapidly changing situations.
• Knowledge of relevant product lines.
• Experience in shared network environments.
• Demonstrated ability to manage large caseloads.
• Experience with electronic documentation systems.
