Overview

The Care Manager plans and manages behavioral and/or physical care with members and works collaboratively with them, their supports, providers, and health care team members.

In Short

  • Advocates and educates beneficiaries on health services and self-management techniques.
  • Conducts assessments to identify barriers and opportunities for intervention.
  • Develops care plans that align with physician’s treatment plans.
  • Generates referrals to providers and community resources.
  • Liaises between service providers to ensure coordinated care.
  • Coordinates with multi-disciplinary health care teams.
  • Evaluates service authorizations for alignment with care plans.
  • Documents encounters in accordance with internal procedures.
  • Assists in identifying opportunities for alternative care options.
  • Contributes to corporate goals through member care plans.

Requirements

  • NYS RN or LCSW or LMSW required.
  • 3 years of work experience in a mental/behavioral health setting.
  • 1 year of pediatric clinical field experience for PEDS positions.
  • Strong interpersonal and assessment skills.
  • Fluency in Spanish or other languages preferred.
  • Experience managing large caseloads in fast-paced environments.
  • Proficient with electronic documentation systems.
  • Knowledge of community health practices for seniors.
  • Ability to relate well with seniors and their families.
  • Experience with Microsoft Excel.
