Behavioral Health Care Management Clinician - Remote

Overview

The Clinical Care Coordinator will provide high quality, integrated, trauma informed and culturally competent care coordination services to members who have complex behavioral health care needs.

• Immediate opening for a full-time Clinical Care Coordinator.

• Coordinate care for members with complex behavioral health needs.
• Develop individualized plans of care.
• Document clinical assessments and case formulations.
• Facilitate communication between members and their support systems.
• Participate in interdisciplinary team meetings.
• Provide technical assistance to community partners.
• Cross cover in the Utilization Management role.
• Work after hours and weekends as needed.

Requirements

• 5 years’ experience in mental health.
• Knowledge of DSM criteria for mental health and substance use disorders.
• Ability to conduct psychosocial assessments.
• Strong communication skills.
• Proficient in Microsoft Office and/or Google Suites.
• Master’s degree required in social work, psychology, or related field.
• Licensed Clinical Social Worker or other Oregon licensed profession required.
• Bilingual in Spanish preferred.
