Housing Substance Use Case Manager - Remote

Overview

The Housing Substance Use (SU) Case Manager (CASAC) provides client-centered substance use counseling and case management to individuals enrolled in the HUD Connection to Housing & Care Program. This role is vital in helping clients achieve housing stability while addressing substance use challenges through individual and group counseling, advocacy, and supportive services.

In Short

• Maintain a caseload of up to twenty-five clients.
• Conduct monthly home visits to assess housing stability.
• Provide individual and group substance use counseling.
• Develop personalized treatment plans for clients.
• Offer comprehensive case management and referrals.
• Advocate for clients in case conferences.
• Ensure accurate documentation of services.
• Monitor and meet all contract service deliverables.
• Participate in team meetings and training sessions.
• Support clients in understanding housing program requirements.

Requirements

• High School Diploma or equivalent required.
• Current CASAC license required.
• Experience in substance use counseling and case management.
• Strong communication and problem-solving skills.
• Ability to work independently and as part of a team.
• Bilingual in English and Spanish preferred.
• Knowledge of housing stability resources.
• Experience with marginalized communities preferred.
• Proficiency in Microsoft Excel and Office Suite.
• Familiarity with government benefit systems.
