Overview

The Hospital Outreach Manager is responsible for building and strengthening relationships with renal discharge planners and case managers to ensure a seamless transition for newly diagnosed and hospitalized dialysis patients to outpatient settings.

Requirements

• Bachelor’s Degree preferred.
