Overview

The Care Coordinator plays a vital role in the healthcare team by facilitating the planning and organizing of critical activities for patients with complex needs. This position requires strong communication and organizational skills to ensure the highest quality care.

In Short

  • Work as part of a team to manage patient care.
  • Coordinate non-clinical screenings and referrals.
  • Assist with patient appointments and documentation.
  • Work hybrid: in-person at clinics and remotely.
  • Flexible schedule with a start time between 7:00 and 8:00 AM.
  • Communicate effectively with patients and healthcare providers.
  • Maintain patient confidentiality and documentation accuracy.
  • Support clinical teams in care management.
  • Must have reliable transportation for commuting.
  • Commitment to service excellence and patient satisfaction.

Requirements

  • High school diploma or GED required.
  • 2 years of experience in a professional office environment preferred.
  • Excellent verbal communication skills.
  • Proficiency with Microsoft Office applications.
  • Experience with Electronic Medical Records preferred.
  • Ability to handle confidential information with discretion.
  • 1-3 years of clinical medical office experience preferred.
  • Experience coordinating patient care is a plus.
  • Experience in direct patient care/support preferred.
  • Ability to work autonomously and in a team setting.
