In Short

• Position is hybrid with occasional telehealth and travel.
• Primary responsibility includes completing Comprehensive Health Evaluations.
• Manage CKD and ESKD patients to improve outcomes.
• Lead the clinical team to ensure coordinated care.
• Full-time, self-managed schedule with no nights or weekends.

• Must have minimum 2 years' NP experience.
• Experience in Value-Based Care preferred.
• Strong clinical judgment and autonomous decision-making required.

Requirements

• Current unrestricted NP license in state of practice.
• DEA license or eligibility within 90 days.
• Current CPR/BLS certification.
• Intermediate computer skills (MS Word, Excel, Outlook).
• Positive, solution-focused attitude required.
• Valid driver's license and insurance.
• 5+ years' experience in relevant fields preferred.
• Experience with Cerner or similar EHR systems is a plus.
