Overview

As an Advanced Patient Care Coordinator, this experienced role requires a thorough understanding of healthcare practices and the ability to apply theory into practice. The individual should independently execute a full spectrum of responsibilities, analyze complex issues, and devise solutions.

In Short

  • Demonstrate proficiency in PCC II skills.
  • Coordinate multifaceted patient care activities.
  • Assist with in-clinic procedures and coordinate with patients.
  • Monitor and report patient's health data.
  • Manage insurance authorizations for visits and medications.
  • Schedule appointments prior to hospital discharge.
  • Fulfill higher-functioning duties as defined by the department.
  • Utilize special software for patient management.
  • Supervise medical devices in clinical settings.
  • Contribute to a patient-centered care experience.

Requirements

  • High School Diploma or GED equivalent.
  • Completion of a medical assisting program or equivalent training.
  • At least four years of overall experience in healthcare.
  • Knowledge of medical terminology and coding regulations.
  • Ability to demonstrate customer service skills.
  • Ability to work independently and as part of a team.
  • Effective communication skills, both verbal and written.
  • Ability to solve problems and identify solutions.
  • BLS certification required.
  • Familiarity with population health concepts.
