• Minimum of 2 years of experience as a Nurse Practitioner required.
• Intermediate computer skills are necessary.
• Home office setup with high-speed internet is required.

Requirements

• Passion for caring for patients with complex, chronic illnesses.
• Willingness to adapt as the program evolves.
• Current, unrestricted NP license in IA, IL, WI, or MN.
• DEA license or eligibility within 90 days.
• Current CPR/BLS certification.
• Strong clinical judgment and autonomous decision-making skills.
• Positive, solution-focused attitude.
• Experience in Value-Based Care or Population Health preferred.
• 5+ years of experience as a Family NP or in related fields preferred.
• Prior experience with Cerner or similar EHR systems is a plus.
