The case manager position is designed to assist individuals in identifying and acquiring both natural and paid supports to live independently in the community.

Requirements

• Bachelor's degree in social work, psychology, sociology, or related field.
• 1 year of experience in social work or case management.
• Valid driver’s license with acceptable driving record.
• Proof of automobile insurance.
• Knowledge of the local community.
• Good oral and written communication skills.
• Understanding of self-directed services.
